• Inform patients and families about the NexaCare program

• Onboard patients post discharge and explain care pathways

• Act as a bridge between hospitals and patients, ensuring seamless coordination

• Manage virtual follow-ups, patient communication, and care continuity

• Identify red flags and ensure timely escalation to clinical teams

🗣 Language Requirements:

• Fluent in Gujarati, English & Hindi (mandatory)
