Job Description
1. Appointment Coordination and Patient Registration
-Schedule, confirm, and manage patient appointments in coordination with providers and clinic schedules.
-Register new and returning patients, ensuring accuracy and completeness of demographic, insurance, and medical information.
-Update patient records in the electronic medical record (EMR) system and verify eligibility prior to visits.
-Provide patients with pre-visit instructions and assist in completing any required forms.
2. Referral Management
-Prepare, initiate, and process medical referrals as requested by providers.
-Ensure referrals are appropriately authorized through the patient’s insurance network.
-Follow up with specialty offices to confirm referral receipt and appointment scheduling.
-Track referral status and update patients and providers accordingly.
3. Diagnostic and Scan Coordination
-Coordinate and track patient diagnostic tests, imaging, and scan reports.
-Ensure timely retrieval and documentation of reports for physician review.
-Communicate test results and next steps to patients as directed by the provider.
-Maintain accurate logs of pending and completed orders to prevent delays in care.
4. Care Coordination and Patient Guidance
-Serve as a primary point of contact for patients with questions regarding their care plans, appointments, or treatment follow-ups.
-Guide patients in scheduling appointments with external facilities, specialists, or labs.
-Coordinate communication between patients, providers, and other healthcare professionals to ensure continuity of care.
5. Insurance and Documentation Support
-Make calls to pt to obtain insurance referral if needed
-Assist patients in completing forms such as FMLA, clearance letters, and other medical documentation as required.
6. Communication and Follow-Up
-Maintain professional and compassionate communication with patients at all times.
-Follow up with patients after appointments to ensure they received appropriate care and understand their next steps.
-Document all interactions and updates accurately in the EMR.
-Escalate any patient concerns, barriers to care, or clinical issues to the appropriate team member or provider.
7. Administrative and Compliance Duties
-Maintain confidentiality and comply with all HIPAA regulations and organizational policies.
-Ensure timely completion of all assigned administrative tasks, including report generation and data entry.
