Key Responsibilities: Care Planning and Coordination:

• Develop personalized care plans for elderly clients based on individual needs, medical history, and preferences.
• Coordinate and manage care services, including medical appointments, therapy sessions, and daily living activities.

Client Advocacy:

• Act as a liaison between elderly clients, their families, and healthcare providers to ensure proper communication and care delivery.
• Address client concerns and resolve issues related to care services promptly.

Team Supervision and Support:

• Oversee the performance of care staff, ensuring adherence to care plans and quality standards.
• Provide training and guidance to care providers on elder care best practices.

Assessment and Monitoring:

• Conduct regular assessments to evaluate clients' physical, mental, and emotional well-being.
• Adjust care plans as necessary based on client needs or health status changes.

Documentation and Reporting:

• Maintain accurate records of care plans, client progress, and staff activities.
• Prepare and submit reports to management or family members as required.

Community and Family Engagement:

• Foster strong relationships with families to ensure they are involved and informed about their loved ones' care.
• Build connections with local resources, healthcare providers, and elder care organizations.

Qualifications and Skills: Must have a two wheeler of his/her own

Education: Bachelor’s degree in Social Work, Nursing, Geriatrics, or a related field (Master’s preferred).

• Experience: 1+ years of experience in elder care, healthcare management, or a similar role.
• Skills: Strong organizational and time-management abilities.
• Excellent communication and interpersonal skills.
• Empathy and understanding of the unique challenges faced by elderly individuals.
• Knowledge of elder care regulations and compliance standards.
• Proficiency in using care management software and tools.
