Pharmacist

Job Title: Pharmacist – Community Pharmacy and Clinical Support

Position Summary

The Pharmacist is responsible for the safe, accurate, and efficient dispensing of medications, patient counseling, and managing inventory within the pharmacy. Additionally, this role acts as a primary liaison to the clinic medical team, assisting physicians with medication therapy management, drug information, patient monitoring, and clinical workflow support.

Core Pharmacy Operations and Dispensing

Point 1: Review, verify, and fill prescriptions with accuracy, ensuring appropriate dosing, contraindications, and drug interactions.

Point 2: Educate patients on correct drug administration, side effects, precautions, storage, and adherence strategies.

Point 3: Maintain stock levels, order supplies, manage controlled substances according to regulatory standards, and handle expiry returns.

Point 4: Ensure full compliance with federal, state, and local pharmacy laws and board of pharmacy practices.

Clinical Support and Physician Collaboration

Point 1: Conduct comprehensive medication reviews for clinic patients, identifying potential drug-related problems and recommending adjustments to the attending physician.

Point 2: Execute physician-delegated clinical tasks, such as tracking lab values relevant to drug dosing.

Point 3: Serve as a clinical resource for physicians, providing evidence-based answers regarding drug selection, dosing in special populations, alternative therapies, and drug shortages.

Point 4: Assist doctors in monitoring patients with chronic conditions through follow-up consultations and adherence checks.

Point 5: Administer vaccines as authorized by clinic protocols or physician standing orders.

Administrative and Interdisciplinary Workflow

Point 1: Maintain clear clinical documentation in both the Pharmacy Management System and the Electronic Health Record system.

Point 2: Communicate directly with clinic medical staff to clarify prescription orders, discuss therapeutic substitutions, or resolve coverage issues.

Point 3: Participate in clinic quality improvement initiatives, medication safety audits, and patient education workshops.

Qualifications and Skills

Point 1: PharmD or Bachelor of Pharmacy degree from an accredited institution.

Point 2: Active and unrestricted Pharmacist license in the jurisdiction of practice.

Point 3: Basic Life Support certification and Immunization certification where applicable.

Point 4: Relevant experience in retail, community, or ambulatory care pharmacy settings.

Point 5: Strong knowledge of pharmacology, clinical pharmaceutics, and therapeutic guidelines.

Point 6: Proficiency in using Electronic Health Records and Pharmacy Management Software.

Point 7: Excellent verbal and written communication skills for patient interaction and inter professional collaboration.

Point 8: Strong attention to detail, problem-solving abilities, and time-management skills.
