Hospital Pharmacist

Key Responsibilities

• Medication Review and Verification: Check physician orders to verify proper drug choice, dosage, route of administration, and safety.
• Ward Rounds and Clinical Care: Attend daily medical rounds with doctors to assess patient responses and recommend tailored pharmacotherapy adjustments (such as antibiotic or pain management changes).
• Compounding and Preparation: Prepare specialized or sterile medications, including intravenous (IV) admixtures, total parenteral nutrition (TPN), and chemotherapy drugs.
• Drug Information and Advisory: Act as an expert resource on drug interactions, side effects, and safe alternatives for physicians, nurses, and patients.
• Inventory and Supply Management: Oversee the procurement, storage, distribution, and stock levels of all hospital medicin
• Patient Counseling and Discharge Planning: Review patient drug charts and discharge summaries to ensure safe medication transitions and educate patients on how to use their prescriptions at home.
• Policy and Formulary Development: Participate in hospital committees to design infection control protocols, manage drug shortages, and maintain the hospital's essential drug list .
