## Position Overview

We are seeking a skilled and caring Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our home care team.

The ideal candidate will provide professional nursing care to patients in their homes, working closely with families and healthcare providers to ensure optimal patient outcomes.

## Key Responsibilities

- Assess patient health conditions and develop individualized care plans

- Administer medications, injections, and treatments as prescribed

- Monitor vital signs and document patient progress

- Perform wound care, catheter care, and other medical procedures

- Educate patients and families on disease management and health maintenance

- Coordinate care with physicians, therapists, and other healthcare professionals

- Maintain accurate and timely documentation in compliance with regulations

- Respond to patient emergencies and provide appropriate interventions

- Travel to patients' homes within the service area

## Required Qualifications

- Current RN or LPN license in Delhi NCR

- CPR/BLS certification

- Minimum 2 years of nursing experience (home care experience preferred)

- Strong clinical assessment and critical thinking skills

- Excellent communication and interpersonal abilities

- Ability to work independently and manage time effectively

- Compassionate, patient-centered approach to care

## Preferred Qualifications

- Previous home health or community nursing experience

- Experience with electronic medical records (EMR) systems

- Specialized training in geriatrics, wound care, or chronic disease management

- Bilingual skills (specify languages if applicable)

- Valid driver's license and reliable transportation

## Physical Requirements
