Job Description
Job Summary:A Hospital Pharmacist is responsible for dispensing medications, ensuring safe and effective drug use, and supporting clinical teams in patient care. The role involves reviewing prescriptions, monitoring drug therapies, and providing guidance to healthcare professionals and patients.
- Key Responsibilities:
- Medication ManagementReview and validate prescriptions for accuracy and appropriateness
- Dispense medications as per physician orders
- Ensure correct dosage, route, and timing of medications
- Patient CareCounsel patients on medication usage, side effects, and interactions
- Monitor patient responses to drug therapy
- Promote safe and rational use of medicines
- Clinical SupportCollaborate with doctors and nurses on treatment plans
- Provide drug information and recommendations
- Participate in ward rounds (in clinical settings)
- Inventory & StorageMaintain proper storage of medicines (including cold chain where required)
- Monitor stock levels and expiry dates
- Ensure availability of essential drugs
- Compliance & SafetyFollow hospital policies and regulatory guidelines
- Maintain accurate records of controlled substances
- Report adverse drug reactions (ADRs)
DocumentationMaintain patient medication records
- Ensure proper billing and documentation of dispensed drugs
- Qualifications & Requirements:Bachelor’s or Master’s degree in Pharmacy (B.Pharm / M.Pharm / Pharm.D)
- Valid Pharmacy Council registration (e.g., PCI in India)
- Knowledge of hospital pharmacy practices and drug regulations
- Skills Required:Strong knowledge of pharmacology and therapeutics
- Attention to detail and accuracy
- Good communication and interpersonal skills
- Ability to work in a fast-paced hospital environment
- Basic computer skills (hospital/pharmacy software)
- Experience: 1–3 years of experience
- Prior hospital or clinical pharmacy experience preferred
- Working Conditions:Rotational shifts (including nights, weekends, and holidays)
- High-responsibility environment involving patient safety
