Job Description

Maintain and update patient medical records (inpatient & outpatient).

Ensure proper filing, indexing, and storage of records.

Retrieve medical records as requested by doctors, staff, or management.

Check records for completeness (doctor notes, reports, discharge summary, etc.).

Handle admission and discharge file documentation.

Maintain confidentiality of patient information.

Assist in medical record audits and reports.

Manage digital records (if hospital uses software/EMR systems).

Prepare and issue medical reports when required.

Track and control movement of patient files.

Coordinate with different departments for missing documents.
