Job Description: Home Care Nurse

Job Summary , The Home Care Nurse provides skilled and compassionate nursing care to patients in their homes. The role focuses on monitoring patient health, administering treatments, coordinating with healthcare professionals, and educating patients and families to ensure safe, comfortable, and high-quality care.

• Key ResponsibilitiesProvide nursing care to patients in their homes as per care plans
• Monitor and assess patient health status, vital signs, and symptoms
• Administer medications, injections, and IV therapy as prescribed
• Perform wound care, catheter care, and other clinical procedures
• Assist patients with post-operative care and chronic condition management
• Coordinate with doctors, hospitals, and family members regarding patient care
• Educate patients and caregivers on medication use, hygiene, nutrition, and disease management
• Maintain accurate and timely patient records and documentation
• Ensure patient safety, comfort, and infection control at all times
• Respond promptly to patient needs and emergencies
