Job Summary:

We are looking for a responsible and organized Home Care Coordinator to manage and coordinate home care services for patients/clients. The candidate will handle staff scheduling, client communication, service quality, and daily operations to ensure smooth home care support.

Key Responsibilities:

Coordinate and manage home care services for clients/patients.

Schedule caregivers, nurses, and support staff according to client requirements.

Maintain regular communication with patients, families, and staff.

Monitor staff attendance, performance, and service quality.

Handle client complaints and resolve issues professionally.

Maintain patient records, reports, and documentation.

Conduct follow-ups with clients regarding care services.

Support recruitment and onboarding of home care staff.

Ensure compliance with company policies and healthcare standards.

Prepare daily, weekly, and monthly operational reports.

Required Qualifications:

Bachelor’s degree or diploma in Healthcare Management, Nursing, Social Work, or related field.

Experience in healthcare, home care, hospital coordination, or patient care management preferred.

Good communication and interpersonal skills.

Basic computer knowledge (MS Office, Excel, email handling).

Strong organizational and problem-solving abilities.
