Job Description

JD - NursePediatric Nurse – Digital Follow-up (Intervention-Led Role)Role Summary“This role is responsible for executing and closing patient interventions generated through continuous monitoring by the system — ensuring no child is left unattended for whom the system raised intervention

Role PurposeTo ensure timely, structured interventions for every child based on system-generated alerts and patient data.

The nurse does not manually diagnose or triage first.

The system identifies need → the nurse executes the right intervention → escalates only when required.

  • Key Responsibilities1. Intervention Tracking (Core Responsibility)Monitor system intervention desk for alerts received for children
  • Track system-generated interventions across:
  • Vitals deviations
  • Medication adherence gaps
  • Symptoms / complaints
  • Activity / feeding concerns
  • Investigations / reports

👉 Your primary job is NOT monitoring — it is ensuring interventions are completed

2. Intervention ExecutionFor every triggered intervention, the nurse must:

  • Take appropriate action within defined timelines
  • Actions may include:
  • Audio Calling / Video Calling / messaging the parent
  • Reinforcing medication or care instructions
  • Asking follow-up questions
  • Guiding next steps (as per protocol)

👉 Focus: No intervention should remain unattended

  • 3. Parent Engagement for ResolutionConnect with parents to:
  • Understand context behind alerts
  • Ensure compliance (medicines, diet, care plan)
  • Provide reassurance and clarity

👉 The goal is resolution, not just response

  • 4. Closure of InterventionsEnsure every intervention is:
  • Addressed
  • Documented
  • Closed in the system
  • Re-open or continue tracking if issue persists

👉 Focus: Intervention lifecycle completion

  • 5. Escalation to PediatricianEscalate only when:
  • Defined clinical thresholds are met
  • No improvement after intervention
  • Parent concern requires doctor input

👉 System + protocol driven escalation (not subjective)

  • 6. Documentation & System UpdatesMaintain accurate records of:
  • Actions taken
  • Parent interactions
  • Outcomes of interventions

👉 Documentation should be clear, structured, and audit-ready

  • 7. Adherence & Continuity SupportEnsure parents follow:
  • Medication schedules
  • Follow-up plans
  • Care protocols

👉 Drive continuity of care, not one-time interactions

  • QualificationsGNM / BSc Nursing (mandatory)
  • Basic understanding of:
  • Pediatric care
  • Common symptoms and medication use
  • Experience1–5 years clinical experience
  • Pediatrics preferred (clinic or hospital)
  • Experience in:
  • Patient follow-up
  • Tele-calling / patient coordination (good to have)
  • Key SkillsAbility to follow structured protocols
  • Strong communication with parents (calm, clear, reassuring)
  • High ownership — does not leave tasks incomplete
  • Attention to detail (tracking multiple patients/interventions)
  • Comfort with digital tools / apps

  • Work SetupCentralized (remote or clinic-based)
  • Works on assigned patient cohorts
  • Defined working hours (e.g., 9 AM – 6 PM)

  • Calling parents
  • App-based communication
