As we expand our healthcare and caregiving services, we are looking for a dedicated and responsible Care Coordinator – Home Healthcare who understands the caregiving ecosystem and can coordinate effectively with nurses, physiotherapists, caregivers, patients, and families.

Role OverviewWe are looking for a healthcare operations professional with experience in elderly care, patient care, post-operative care, palliative care, physiotherapy support coordination, and home-based caregiving services.

The candidate will be responsible for understanding patient and family requirements, coordinating with staff nurses, nursing assistants, physiotherapists, caregivers, and other care professionals, and ensuring smooth service delivery for clients.

This is an office-based coordination and operations role. The person should be comfortable speaking to families, understanding care requirements, coordinating with healthcare professionals, managing caregiver availability, and ensuring timely placement and service follow-up.


Key Responsibilities:


1. Client Requirement Understanding:

  • Speak with families and understand their care requirements in detail.
  • Identify whether the requirement is for elderly care, post-operative care, palliative care, bedridden patient care, dementia care, physiotherapy support, general caregiving, or nursing support.
  • Ask the right questions to understand the patient’s condition, mobility level, medical dependency, food habits, language preference, duty timings, and family expectations.
  • Clearly document all client requirements in the internal system or tracker.


2. Caregiver and Healthcare Staff Coordination:

  • Coordinate with staff nurses, nursing assistants, GDA candidates, ANM/GNM candidates, physiotherapists, and caregivers.
  • Check availability, experience, location preference, language skills, and suitability of care staff.
  • Match the right caregiver or healthcare support staff based on client needs.
  • Brief caregivers and nurses properly before assigning them to a client.
  • Ensure the assigned staff understands the patient condition, family expectations, hygiene standards, duty timings, and responsibilities.


3. Home Healthcare Operations:

  • Manage day-to-day coordination between clients, caregivers, nurses, physiotherapists, and internal teams.
  • Follow up with clients after placement to ensure service satisfaction.
  • Support in resolving issues related to caregiver behavior, attendance, duty understanding, replacement needs, or service gaps.
  • Coordinate replacement arrangements whenever required.
  • Ensure continuity of care and avoid service disruption for clients.


4. Healthcare Knowledge and Care Planning:

  • Understand basic care requirements for:
  • Elderly care
  • Post-operative care
  • Palliative care
  • Bedridden patient support
  • Stroke recovery support
  • Dementia and Alzheimer’s care
  • Mobility support
  • Physiotherapy coordination
  • Patient hygiene and feeding support
  • Medicine reminder support
  • Hospital-to-home transition care
  • Help families choose the right level of care support based on the patient’s condition.
  • Escalate medical or clinical concerns to qualified professionals when needed.


5. Staff Screening and Profiling:

  • Speak to caregivers, nursing assistants, staff nurses, and physiotherapists to understand their experience.
  • Profile candidates based on skills, past cases handled, language, location, salary expectation, shift preference, and availability.
  • Maintain an updated database of healthcare and caregiving staff.
  • Support the team in identifying reliable and suitable care professionals for different types of cases.


6. Client Communication and Service Follow-up:

  • Maintain polite, clear, and professional communication with clients and families.
  • Give realistic service updates without overpromising.
  • Coordinate with the sales and operations team for closures and service delivery.
  • Handle client concerns with empathy, patience, and ownership.
  • Build trust with families who are dealing with sensitive healthcare and elderly care situations.


7. Documentation and Reporting:

  • Maintain accurate records of client requirements, staff profiles, placements, follow-ups, complaints, replacements, and service status.
  • Prepare daily or weekly reports on active cases, pending requirements, staff availability, and escalations.
  • Update internal trackers regularly.
  • Ensure proper handover of important case details to the relevant internal team members.


Required Experience:

The ideal candidate should have 2–3 years of experience in any of the following areas:

  • Home healthcare operations
  • Elderly care coordination
  • Patient care services
  • Nursing bureau coordination
  • Caregiver placement services
  • Hospital discharge coordination
  • Physiotherapy service coordination
  • Palliative care support services
  • Healthcare customer support
  • Medical staffing or nursing staff coordination

Experience in speaking with staff nurses, physiotherapists, nursing assistants, caregivers, and patient families will be strongly preferred.


Required Skills:

  • Good understanding of home healthcare and caregiving services.
  • Ability to understand patient and elderly care requirements.
  • Strong communication skills in English and Hindi. Kannada will be an added advantage.
  • Ability to coordinate with caregivers, nurses, physiotherapists, and families.
  • Strong follow-up and documentation skills.
  • Empathy and patience while handling families and elderly care cases.
  • Ability to handle sensitive healthcare conversations professionally.
  • Good judgment in matching the right caregiver to the right family.
  • Comfortable working from office for 6 days a week.
  • Basic computer knowledge, including Google Sheets, Excel, WhatsApp, CRM, or internal tracking tools.
  • Preferred Candidate ProfileCandidate from healthcare, home healthcare, nursing bureau, elderly care, rehabilitation, physiotherapy coordination, or patient care services background.
  • Someone who has dealt with elderly patients, bedridden patients, post-operative cases, or palliative care cases.
  • Someone who understands the importance of trust, reliability, hygiene, discipline, and empathy in caregiving.
  • A person who can balance client expectations with practical staff availability.
  • Someone who is calm under pressure and can solve operational issues quickly.


Education QualificationAny of the following qualifications will be preferred:

  • Graduate in any discipline
  • Healthcare management background
  • Nursing-related qualification
  • GDA, ANM, GNM, B.Sc Nursing exposure
  • Hospital administration or healthcare operations background


Formal medical qualification is not mandatory, but healthcare or caregiving industry experience is important.
