Care Manager

Location: Indira Nagar, Maleshwaram, Rajaji Nagar, HAL  Roles & Responsibilties of Care Manager

1. Be responsible for the assigned families within the purview of the job. Manage end to end responsibility of handling requests of customers assigned.

2. Responsible for seamless Member onboarding including App Installation and demo. Also familiarize customer on triggering Emergency .

3. Visit families as per the monthly schedule or whenever requested by the family to help with errands like grocery, medicine, bank related work , visit to religious places etc.

4. Ensure that the requests by the customer is fulfilled. (Requests within scope of work).

5. Accompany elders for doctor visit.

6. PMS handling and settlement within 48 hours.

7. Coordinate third party services with central office and ensure timely services to customer.

8. Emergency handling as per Protocol.

9. Ensure beneficiaries are comfortable in their space.

10. Responsible for 100% retention of customer excepting unavoidable situations.

11. Ensure timely updates to the company and the customers on the status of the work.

12. Display patient and compassionate behaviour with senior citizens

13. Make a clear schedule of visits for the week. Each visit request to be created before start and ended as soon as visit completed.

14. Set reminders for customers follow-up on the calendar.

15. Respond completely and clearly in the family and other official group.

16. Summarize visit and post in the family group / update the sponsor. Every visit to be mandatorily summarized.

17. Maintain financial discipline while handling PMS.

18. Keep reporting manager posted about the families on regular basis. Any suspicion should be got to the notice of Reporting Manager and Central office.

19. Responsible for getting consent form filled and uploaded during placement of third-party services.
